Designed for individuals who require ongoing reassurance and structured follow-up after discharge. Includes structured wellbeing visits and close monitoring of recovery. Additional visits( for example, 2-3 visits per week) can be arranged depending on individual needs. Pricing starts from £950 and increases based on the frequency, and level of ongoing support.

Fully insured. Enhanced DBS checked. Nurse-led non-clinical discharge support service.
